As a member of our hospice-centered team, you will engage with clients and their families from the comfort of your own home. • Conduct comprehensive assessments for clients and develop tailored treatment plans to address their unique needs.
• Provide individual, group, and family counseling sessions, utilizing evidence-based approaches to promote healing and coping skills.
• Monitor clients' progress towards their treatment goals, adjusting strategies as necessary to support their evolving needs.
• Collaborate with other mental health and medical professionals, including psychiatrists, psychologists, and hospice care teams, to deliver holistic care.
• Facilitate crisis intervention and support clients experiencing high-stress situations, ensuring their immediate emotional safety.
• Maintain meticulous records of client interactions and treatment histories, adhering to confidentiality and privacy standards.
• Stay current with the latest research, methodologies, and trends in mental health and hospice care to continuously improve service delivery.

Your role will involve working closely with clients who are facing life-limiting illnesses, grief, and significant transitions. You will address their mental health needs alongside providing crucial emotional support for families navigating complex emotions during hospice care. This position emphasizes compassion, empathy, and the ability to foster trusting relationships, which are essential for effective therapeutic engagement.

• A Master's degree in Social Work from an accredited institution.
• A valid and current state license to practice as a clinical social worker.
• Demonstrable strong communication and interpersonal skills that facilitate constructive relationships with clients and team members.
• The capacity to work autonomously while also being a collaborative team player.
• Strict adherence to confidentiality and sensitivity when managing personal client information.

• Previous experience in hospice care or end-of-life counseling is highly desirable.
