This position is fully remote, offering you the flexibility to work from your chosen environment while still being an integral part of our collaborative team. The work setting involves frequent communication with clients over virtual platforms to facilitate assessments and deliver counseling. Although the primary role is remote, engagement with interdisciplinary teams—including healthcare professionals, caregivers, and families—is essential to ensure comprehensive care coordination. Our regular work schedule is from Monday to Friday, 8:00 AM to 4:30 PM, with some flexibility required for evening or weekend appointments.

• Conduct thorough assessments of seniors' psychosocial needs, employing a holistic approach that includes emotional, mental, and social factors.
• Develop and implement personalized care plans alongside clients and their families, focusing on actionable steps to achieve specific goals.
• Provide ongoing counseling sessions to support seniors facing significant life transitions or mental health challenges.
• Advocate on behalf of clients to help them access appropriate community resources, ensuring they receive crucial support services.
• Collaborate closely with interdisciplinary teams—such as nurses and healthcare professionals—to enhance client support and monitor overall well-being.
• Regularly check in with clients through virtual visits, assessing their condition and providing necessary interventions to address health or safety concerns.
• Facilitate open communication among family members regarding care strategies and emotional support, aiming to resolve conflicts and enhance relationships.
• Educate clients and families about available community programs, support groups, and resources to improve social engagement and overall quality of life.
• Maintain meticulous documentation of client interactions, care plans, and assessments within our electronic health record (EHR) system, ensuring compliance with all regulations.
• Stay updated on best practices in social work, continually enhancing capabilities through self-study and attending relevant training sessions.

This role primarily focuses on seniors within the Bakersfield, California area, understanding their specific mental health challenges, including anxiety, depression, and adjustment disorders related to aging. Our aim is to empower these clients and their families by providing comprehensive support tailored to their unique needs. By using an empathetic and individualized approach, you will help clients regain control over their mental health while enhancing their emotional well-being.

• Master's degree in Social Work from an accredited institution.
• Current and valid state licensure as a Clinical Social Worker (LCSW).
• A minimum of two years of relevant experience in social work, particularly within a community or geriatric setting.
• Thorough familiarity with social work principles, aging-related issues, and local community resources.
• Strong assessment and counseling skills, coupled with excellent problem-solving abilities.
• Proficient at building rapport with seniors and their families, demonstrating empathy and compassion.
• Exceptional communication and interpersonal skills, facilitating effective collaboration with a multidisciplinary team.
• Proficiency in electronic health record (EHR) systems and common office software.

• Experience specifically in telehealth or remote counseling can enhance the ability to connect with distanced clients.
