Work Environment

This position is fully remote, offering you the flexibility to work from your chosen environment while still being an integral part of our collaborative team. The work setting involves frequent communication with clients over virtual platforms to facilitate assessments and deliver counseling. Although the primary role is remote, engagement with interdisciplinary teams—including healthcare professionals, caregivers, and families—is essential to ensure comprehensive care coordination. Our regular work schedule is from Monday to Friday, 8:00 AM to 4:30 PM, with some flexibility required for evening or weekend appointments.

Key Responsibilities
  • Conduct thorough assessments of seniors' psychosocial needs, employing a holistic approach that includes emotional, mental, and social factors.
  • Develop and implement personalized care plans alongside clients and their families, focusing on actionable steps to achieve specific goals.
  • Provide ongoing counseling sessions to support seniors facing significant life transitions or mental health challenges.
  • Advocate on behalf of clients to help them access appropriate community resources, ensuring they receive crucial support services.
  • Collaborate closely with interdisciplinary teams—such as nurses and healthcare professionals—to enhance client support and monitor overall well-being.
  • Regularly check in with clients through virtual visits, assessing their condition and providing necessary interventions to address health or safety concerns.
  • Facilitate open communication among family members regarding care strategies and emotional support, aiming to resolve conflicts and enhance relationships.

Qualifications
  • Master's degree in Social Work from an accredited institution.
  • Current and valid state licensure as a Clinical Social Worker (LCSW).
