You will collaborate with a dedicated interdisciplinary team through innovative telehealth technologies, enabling you to provide effective support and intervention regardless of location.

• Utilize electronic medical records to monitor, document, and communicate patient progress against established goals, ensuring adherence to the social work plan.
• Engage in proactive collaboration with an interdisciplinary team, anticipating patient needs and fostering effective communication to enhance the plan of care.
• Offer support and information to patients and their families, particularly concerning transition plans and continuity of care.
• Maintain up-to-date knowledge of government and private payer networks to ensure accurate transitions and resource referrals.
• Advocate for patients' rights, providing necessary support to access protective services and navigating complex healthcare landscapes.
• Stay alert to and report any signs of vulnerability or suspected abuse, in accordance with legal obligations.
• Assist in care systems to prevent patient readmissions by participating in strategic planning processes and utilizing relevant intervention technologies.
• Plan and facilitate transition conferences with patients and their families to ensure effective communication and support during critical moments.
• Identify and manage at-risk patients using cutting-edge tools and technologies, ensuring a comprehensive clinical handoff.
• Provide additional counseling and support as needed, addressing financial or social concerns that hinder patient well-being.
• Complete other duties as assigned to support departmental objectives and patient care initiatives.

Our clientele consists of diverse individuals dealing with various mental health challenges exacerbated by physical illnesses, including acute, chronic, or terminal conditions. You will work directly with patients and their families, equipping them with essential tools and knowledge to manage their health effectively. Your compassionate approach will help create a safe space for clients to express their concerns and work toward recovery.

• A Master's or Doctorate degree in clinical psychology, counseling, or clinical social work (e.g., MA, MS, MSW, MFT, MHC, PhD, PsyD) from an accredited institution.
• A valid independent license in your respective field in the state of Oregon (LCSW, LMFT, LPC).
• Demonstrated ability to manage a significant caseload while interacting effectively with various healthcare providers and support staff.
• Proficiency in using electronic medical record systems and strong email communication skills.

The position offers a competitive salary ranging from USD 100,000 to 105,000 annually, commensurate with experience and qualifications.
