This is a remote position, allowing you to deliver therapeutic services from the comfort of your own location. Although you will primarily work from home, you will be integral to the health clinic's collaborative environment, closely coordinating with primary care providers and healthcare teams through secure telehealth platforms. • Work collaboratively with primary care providers to identify and manage mental health and psychosocial issues, integrating your expertise into the broader healthcare team.
• Conduct thorough patient assessments, focusing on diagnostic evaluations and functional needs, to develop and recommend tailored treatment plans.
• Provide targeted interventions for patients requiring mental health support, employing evidence-based therapeutic techniques to drive progress.
• Guide primary care providers in making informed decisions regarding patient referrals to specialized services when necessary.
• Implement follow-up procedures to continually monitor patient progress, making adjustments to treatment as required to achieve optimal outcomes.
• Develop individualized relapse prevention strategies and assist patients in maintaining their mental and emotional stability.
• Identify at-risk populations and create proactive plans to mitigate the progression of mental health issues within the community.
• Document patient progress and diagnosis accurately in treatment charts, maintaining compliance with confidentiality standards.
• Facilitate open communication with primary care providers, ensuring collaborative approach to patient care and treatment plan effectiveness.
• Provide support to patients adhering to prescribed medical treatments, including guidance on coping with any associated side effects.
• Support clinic management and team members by offering insights on behavioral health services and exploring opportunities for program evaluation.
• Strictly adhere to the professional Code of Ethical Conduct and uphold the values of our organization in all interactions.

Our clinic serves a diverse patient population, primarily comprising veterans and their families. We are dedicated to addressing various mental health conditions, such as anxiety, depression, PTSD, and other behavioral health-related issues. • A Master’s Degree in Clinical Counseling is required.
• Must hold a current LPC (Licensed Professional Counselor) License in Arizona.
• In-depth knowledge of mental health assessment techniques, treatment planning strategies, and therapeutic methodologies, including cognitive behavioral therapy and family systems therapy.
• Preferred bi-lingual proficiency in English and Spanish to enhance communication and service delivery.
• Possess the mental and physical health necessary to meet the day-to-day demands of this counseling position.
• A thorough understanding of the social and personal impacts of mental and physical health issues on individuals and families.
• Familiarity with community resources and organizational structures that support mental health and wellness.

• Experience in a community health setting, particularly in integrated healthcare, is highly desirable.
• Prior experience working with veterans and their families is a significant plus.
• Demonstrated skills in teletherapy or remote counseling approaches are preferred.
