• Conduct thorough psychosocial assessments to understand each patient's mental health concerns, strengths, and resources, ensuring a compassionate approach to care.
• Develop individualized treatment plans that address both emotional and psychological needs, incorporating evidence-based therapies such as cognitive-behavioral therapy and trauma-informed care.
• Provide individual, family, and group therapy, actively engaging both patients and their loved ones in the healing process, especially during times of crisis.
• Facilitate psychoeducation classes and support groups to empower patients and families with coping strategies and resources.
• Collaborate with interdisciplinary teams, including nurses, physicians, and other healthcare professionals, to coordinate comprehensive patient care tailored to individual circumstances.
• Implement crisis intervention strategies and safety planning as necessary, addressing urgent mental health issues effectively.
• Maintain accurate and confidential documentation in accordance with ethical and legal standards, ensuring compliance and continuity of care.

This role primarily focuses on hospice patients, who may be experiencing complex mental health challenges, including anxiety, depression, and existential distress. You will work closely with terminally ill individuals, providing support that promotes dignity, peace, and understanding in their final stages of life. Additionally, you will extend your expertise to families, helping them navigate grief and loss, which is a significant aspect of hospice care.

• A Master’s degree in Social Work from an accredited program.
• Current active state licensure as a Licensed Clinical Social Worker (LCSW).
• Minimum of two years of relevant experience in mental health or hospice settings, demonstrating a clear understanding of end-of-life care challenges.
• Knowledge and practical experience with evidence-based therapeutic modalities, particularly those applicable to grief and loss.
• Strong interpersonal and communication skills, with the ability to build rapport and trust with vulnerable populations.

• Experience working with interdisciplinary teams in care settings, especially those focusing on geriatric or terminally ill patients.
• Training in trauma-informed care and bereavement counseling.
• Familiarity with electronic health record (EHR) systems for comprehensive patient documentation and care coordination.
