• Conduct thorough, ongoing assessments to identify the unique needs of each patient, developing personalized care plans.
• Facilitate effective communication between various stakeholders, including supervisors, families, and healthcare providers, to discuss patient progress and make necessary adjustments to treatment plans.
• Organize and maintain a tentative schedule of appointments, home visits, and monitoring sessions to ensure timely support for your clients.
• Coordinate and perform home visits as needed to observe patients in their natural environment, gathering essential data for treatment evaluations.
• Implement and monitor the effectiveness of treatment plans to ensure they are fully integrated into the patient’s overall care approach.
• Actively participate in interdisciplinary team meetings, contributing insights and strategies that enhance patient care across the organization.
• Prepare detailed monthly and quarterly progress notes to document patient advancements and maintain compliance with regulatory standards.
• Develop and facilitate training sessions aimed at ensuring team members are thoroughly trained in program protocols and documentation requirements.
• Engage in follow-up interactions with clients through interviews and observations, providing additional support as necessary.
• Maintain accurate and confidential records to ensure all relevant information is easily accessible to authorized personnel.
• Create strong relationships with the community and mental health networks to support the development and growth of our services.

As a Licensed Professional Counselor at Mental Thrive, you will work with a diverse clientele, including both adults and youth experiencing a range of mental health challenges. By employing evidence-based practices, you will support individuals dealing with anxiety, depression, trauma, substance misuse, and family dynamics. Your efforts will be crucial in helping clients develop coping strategies and improve their overall emotional well-being.

• A Master's or Doctoral degree in Counseling, Psychology, Social Work, or a related field is essential.
• Active and unrestricted licensure as a Professional Counselor (LPC) in the state of Arizona is required.
• A minimum of two years of counseling experience, preferably in a clinical setting, is necessary for this role.
• A background in specialized therapy areas such as family therapy, couples therapy, trauma-informed care, or substance misuse treatment is preferable.
• Comprehensive knowledge of legal and ethical standards pertinent to mental health counseling.
• Exceptional interpersonal and communication skills to effectively interact with clients, families, and colleagues.
• Demonstrated ability to work collaboratively with a healthcare team comprising various disciplines.
• Familiarity with electronic medical records (EMR) software and related technologies is required.
