Key Responsibilities
  • Conduct thorough psychosocial assessments by engaging in interviews, observations, and reviewing medical records to evaluate patients' emotional, social, and environmental circumstances influencing their recovery.
  • Provide individualized counseling and emotional support to students and their families, facilitating adjustment to their conditions and enhancing their overall well-being.
  • Collaborate closely with a multidisciplinary rehabilitation team to formulate tailored treatment plans that address each student's psychosocial needs and goals.
  • Assist students in navigating and accessing community resources such as support groups, vocational rehabilitation programs, or housing assistance, ensuring that they receive comprehensive care.
  • Educate families on available resources and post-rehabilitation services, helping them understand the journey ahead after discharge.
  • Play an active role in discharge planning that includes facilitating transitions home or to alternative care settings, ensuring continuity of care and support.
Patient Population / Client Focus

You will primarily work with students from diverse backgrounds who are experiencing challenges related to rehabilitation, including but not limited to those recovering from mental health issues, developmental delays, and physical disabilities. Your expertise will provide essential support to help these students not only regain independence but also integrate back into their educational settings, fostering a nurturing environment conducive to their academic and social success.

Qualifications
  • A Master’s degree in Social Work from an accredited program is required.
  • Current and active state licensure as a Licensed Clinical Social Worker (LCSW).
  • A strong understanding of adjustment issues, coping mechanisms, and holistic care principles is essential.
  • Exceptional communication and interpersonal skills to effectively engage with patients, families, and fellow team members.
Preferred Experience
