Work Environment

Key Responsibilities
  • Conduct thorough psychosocial assessments by engaging patients in interviews, observing behavior, and reviewing relevant medical records to identify emotional, social, and environmental factors affecting their recovery.
  • Provide empathetic counseling and support to patients and their families, helping them address adjustment issues related to their health conditions and emotional well-being.
  • Collaborate with a multidisciplinary rehabilitation team to craft individualized treatment plans, ensuring that the patients' psychosocial needs are at the forefront of care.
  • Assist patients in accessing community resources, such as support groups, vocational rehabilitation services, or housing assistance, to help them transition successfully back into the community.
  • Educate patients and their families about available community resources and post-rehabilitation services, ensuring they feel empowered and informed throughout their recovery journey.
  • Participate actively in discharge planning and facilitate smooth transitions for patients moving back home or to other care environments.
Patient Population / Client Focus

As a Licensed Clinical Social Worker at Thrive Compassionate Counseling, you will engage with a diverse patient population. Qualifications
  • A Master's degree in Social Work from an accredited program is required.
  • You must hold an active state license as a Licensed Clinical Social Worker (LCSW).
