Key Responsibilities

• Conduct thorough assessments to determine the mental health needs of clients, utilizing established frameworks and methodologies to gather comprehensive client histories.
• Develop, implement, and adjust personalized treatment plans based on effective evidence-based therapeutic modalities, ensuring the mental and emotional well-being of clients.
• Facilitate individual, couples, and family therapy sessions within a virtual framework, offering a safe space to explore complex emotions related to grief, loss, and end-of-life considerations.
• Collaborate closely with a multidisciplinary healthcare team including physicians, nurses, and chaplains to provide holistic care that addresses both mental and physical health needs.
• Maintain meticulous and confidential records in accordance with established legal and ethical standards while documenting client progress and treatment effectiveness.
• Engage in ongoing professional development, attending workshops and training to refine clinical skills and stay updated on best practices in social work and hospice care.
• Monitor and evaluate the effects of interventions on client outcomes, adjusting treatment strategies in response to client feedback and progress.

Patient Population / Client Focus

You will have the opportunity to work with a diverse range of clients, particularly individuals and families facing significant life transitions due to terminal illness. Your role will focus on providing psycho-social support, addressing issues related to grief, trauma, and adjustment, ensuring they receive the empathetic care they deserve during this challenging time.

Qualifications

• A Master’s degree in Social Work from an accredited program, equipping you with the foundational knowledge necessary for clinical practice.
• Current active state licensure as a Licensed Clinical Social Worker (LCSW) in Colorado.
• A minimum of three years of clinical experience in a mental health or outpatient setting, with a strong emphasis on working with diverse populations, particularly in hospice or palliative care contexts.
• Robust knowledge and experience with evidence-based therapeutic modalities, including Cognitive Behavioral Therapy (CBT), Dialectical Behavioral Therapy (DBT), and grief counseling techniques.
• Exceptional interpersonal, communication, and assessment skills to facilitate effective therapeutic relationships with clients and families.

Preferred Experience

• Experience in hospice or palliative care settings, demonstrating a clear understanding of the unique emotional and psychological needs of terminally ill patients.
• Familiarity with electronic health record systems and telehealth practices, allowing for effective client interactions and documentation.
• A demonstrated commitment to ongoing professional development and staying current with mental health trends and research.

Compensation and Benefits
