Job Objective:ย ย 

The Triage RNโ€™sย objectiveย is to provide sound clinical communication and problem solving through a strong ability to triage on site situations, incidents, and staff questions remotely through both verbal and telephonic meansย and at a high volume of incoming calls.ย ย Additionally, Triage nurses will be making outbound calls to Hospice, Home Care, Families, and updating provider portals via secured link.ย 

Qualifications:ย 

  • Must have a current RN license in good standing in the state of MNย and any other state that ACS is providing services.ย ย 
  • Must have a minimum ofย 7ย years of experience as an RN/LPN/CNAย or combined in Assisted Living, Memory Care or LTC.ย 
  • Great organizations skills with the ability to hold self-accountable when not directly supervised.ย 
  • Exceptional communications skills both verbal and writtenย ย 
  • Experience with Electronic Healthย Records i.e., Eldermark, Epic or Point Click Care, Residex and others.ย 
  • Experience or a willingness to learnย state of the artย phone and telecommunication technologyย 
  • Must have a good understanding of Pharmacology, Blood Glucose Monitoring, firstย aid,ย  highย acuity LTC nursingย and emergency preparedness.ย ย 
  • Must be professional and able to remain calm inย difficult situationsย 
  • Must have a willingness to work with your team to manage time off and open shifts.ย 

Job Responsibilities:ย 

  • Answer calls when scheduled to be on duty including nights, weekends, and holidays.ย 
  • Must have great attention to detail and clinical investigative skillsย 
  • Document appropriately in the EHRย

  • Must be able to follow and adhere to our work from home policies without varianceย 
  • Communicate professionally both in oral and written formย whileย maintainingย HIPAAย complinaceย

  • Keep up with AL educational and mandatory state requirements.ย ย 
  • Self-Manage and track all various state licenses and CEU requirementsย 
  • Attendance of Mandatory Team Meetings/Training meetings outside of your work schedule.ย ย

  • Must be able to remain awake and alert during overnight hoursย 
  • Must not consume alcohol or illegal substances while on duty or 12 hours prior to your shiftย 
  • Must be able to input information into anย EHRย 
  • Must be able to work everyย other weekendย and everyย other Holidayย

  • This is a work from home position. Youย must have a quiet private space free from pets,ย familyย and background noise.ย

Working Hours:ย 

  • Evenings and overnights M-Fย 
  • We offer aย varietyย of shifts on Weekendsย and Holidaysย for 24/hour coverageย based on client need and call volumes.ย
