This position allows social workers to operate remotely, offering flexibility to manage their schedules while ensuring quality interactions with clients through video sessions and traditional outpatient appointments as necessary. • Conduct individual counseling sessions through video conferencing platforms, addressing the psychosocial needs of clients and their families in hospice care.
• Perform comprehensive assessments of clients' emotional and psychological needs, determining appropriate intervention strategies and establishing tailored treatment plans.
• Facilitate open discussions that allow patients and families to address grief, fear, and anticipatory loss, ensuring they feel empowered and supported throughout their journey.
• Collaborate with a multidisciplinary team including medical staff, nurses, and other healthcare professionals to coordinate care and provide a holistic support network for patients.
• Advocate for patients by helping them set and achieve personal goals related to coping with illness, loss, and end-of-life matters.

• A Master’s degree in Social Work from an accredited program is required.
• Current and valid state licensure as a Licensed Independent Clinical Social Worker (LICSW) with no history of disciplinary actions.
• Proven experience in clinical social work practice, specifically in areas related to assessment, diagnosis, and developing effective treatment strategies.
• Exceptional communication skills with a demonstrated ability to build rapport with diverse clients, including those in emotional distress.

Hope for Mental Health Services offers a competitive compensation package, with salaries ranging from $52,000 to $70,000 annually, commensurate with experience and qualifications.
