How do remote telehealth nurse practitioners typically manage patient care coordination while working virtually?

Remote Telehealth Nurse Practitioners (NPs) often rely on secure digital platforms to communicate with patients, document care, and collaborate with other healthcare professionals. They coordinate care by scheduling virtual follow-ups, sharing electronic health records, and referring patients to in-person providers or specialists when necessary. Building strong communication skills and being comfortable with technology are essential, as much of the collaboration with physicians, pharmacists, and support staff happens through messaging, video conferences, and shared documentation tools.
