Responsibilities
The Collaborating Physician works with an APN (or more than one) who is performing in home and/or telehealth wellness assessments on patients' designated by HarmonyCares Medical Group. APNs working with the Collaborating Physician will not have prescriptive authority, nor will they be treating patients. As a Collaborating Physician you will:
  • Be available via telecommunication or in-person for consultation with APN to address patient assessment questions
  • Regularly performs annual chart reviews
  • Consults with APN ensure adherence to policies, procedures, state laws and quality
  • Be accountable for outstanding customer service to all external and internal customers

Qualifications
  • Active/unrestricted Medical Physician License
  • Board certified/board eligible required
  • Residency completion required
  • 1 year experience in Healthcare management
  • Experience with Risk Adjustment
  • Must reside in Indiana
  • Must maintain a valid driver's license and good driving record
