• Conduct thorough assessments of clients’ mental health and social support needs through virtual interviews, identifying areas for intervention and support.
• Develop and implement individualized treatment plans that are client-centered and goals-focused, ensuring alignment with best practices in social work.
• Coordinate with healthcare providers, community organizations, and other stakeholders to advocate for clients’ rights and facilitate access to necessary resources, services, and support.
• Utilize telehealth platforms and other technology to deliver therapy sessions, manage case documentation, and monitor client progress effectively.
• Participate in regular team meetings to discuss client cases, share insights, and collaborate on care strategies that promote holistic health outcomes.
• Offer crisis intervention and support as needed, employing evidence-based techniques to help clients navigate challenging situations.

The patients we serve come from a broad spectrum of backgrounds, requiring diverse approaches based on cultural, socioeconomic, and psychological factors. As a Remote LCSW at Breathe Easy, you will work with individuals and families facing mental health challenges, emotional distress, and social barriers that inhibit their ability to thrive. Your role will directly impact their resilience and coping strategies, enhancing their overall well-being.

• A Master’s Degree in Social Work (MSW) from an accredited institution.
• Current Oregon state licensure as a Licensed Clinical Social Worker (LCSW).
• A minimum of 3 years of professional experience in social work, preferably in mental health or community health settings.
• Demonstrated ability to communicate effectively and empathetically with clients and colleagues.
• Proficient with remote work technologies, teletherapy platforms, and case management software.

• Experience working in a community health clinic or similar environment with a focus on holistic care.
• Familiarity with evidence-based practices in social work, including motivational interviewing and cognitive-behavioral techniques.
• Skills in crisis intervention and trauma-informed approaches to care.
