You will be interacting primarily with seniors who may require varying levels of support and assistance. Your responsibilities will involve virtual assessments, counseling sessions, and ongoing communication, ensuring a continuous relationship with clients and their families. You will need to effectively navigate digital platforms and remote technologies to connect with clients and other team members.

• Perform in-depth assessments of seniors' psychosocial conditions to identify their emotional, mental, and social needs.
• Collaborate closely with clients and their families to develop individualized care plans that align with their unique goals and preferences.
• Provide counseling and emotional support to seniors navigating life transitions, such as loss, illness, or isolation.
• Advocate on behalf of clients, ensuring their rights are respected and helping them access community resources and services.
• Engage with interdisciplinary teams, including nurses, caregivers, and other healthcare professionals, to coordinate holistic care.
• Conduct regular virtual check-ins to monitor clients' well-being, reassess their needs, and address potential safety issues.
• Facilitate family communication, helping to mediate conflicts and enhance family collaboration in care plans.
• Educate seniors and their families about available community resources, support groups, and programs to promote social engagement.
• Maintain accurate and up-to-date documentation regarding assessments, care plans, and progress notes in the electronic health record (EHR) system.
• Stay informed about current social work practices, ethical standards, and relevant regulations to ensure compliance.

This role specifically focuses on serving seniors, which may include individuals facing varying degrees of physical and cognitive decline, social isolation, or emotional distress. Many of our clients are experiencing major life changes, such as transitioning to assisted living or coping with bereavement. Your expertise in serving this demographic will be crucial in promoting their emotional resilience and overall well-being.

• Master's degree in Social Work (MSW) from an accredited university.
• Current and valid licensure as a Master Social Worker (LMSW) in the state of Texas.
• A minimum of two years of direct social work experience, preferably within a community center or geriatric setting.
• In-depth knowledge of social work principles, community resources, and challenges specific to the aging population.
• Exceptional assessment and counseling skills with a focus on empathetic communication.
• Ability to work collaboratively and effectively communicate within an interdisciplinary team.
• Proficient in using electronic health record systems and relevant computer applications for documentation and communication.

• Experience working with seniors or within a community-based healthcare environment.
• Familiarity with telehealth platforms and technologies.
• Background in crisis intervention or conflict resolution.
