This full-time role allows for a remote work environment while providing pivotal mental health services to a diverse patient base with complex medical and psychiatric needs.

We specialize in addressing the unique challenges faced by patients with co-occurring mental health and medical conditions, such as chronic pain syndromes, autoimmune disorders, and substance use disorders. Our multidisciplinary approach ensures patients receive comprehensive support from both mental health and medical professionals.

In this remote role, you will work from the comfort of your home while remaining closely connected with our outpatient office team. Our operational hours are Monday through Friday, 8:00 AM to 5:00 PM, with occasional evening shifts to accommodate patient needs flexibly. • Conduct thorough mental health assessments to evaluate patients' psychological status while considering the impact of their medical conditions.
• Develop comprehensive and integrated treatment plans tailored to address both psychiatric and medical needs.
• Provide individual and group therapy sessions, employing evidence-based strategies that resonate with the unique circumstances of each patient.
• Collaborate closely with medical providers to ensure coordinated care that leads to effective treatment outcomes.
• Implement crisis intervention protocols and provide support during mental health emergencies.
• Educate patients about the interplay between mental health and physical health, equipping them with the tools to manage their wellness journey effectively.
• Maintain precise and confidential documentation in alignment with legal and ethical standards, ensuring all patient records are up-to-date.
• Actively participate in team meetings, case conferences, and interdisciplinary care planning sessions to enhance integrated care delivery.
• Foster professional growth by staying abreast of current research, best practices, and innovations in integrated care methodologies.

This role primarily focuses on patients experiencing a range of co-occurring mental and medical conditions. Your expertise will help support individuals managing chronic illnesses alongside mental health challenges, promoting their overall well-being and encouraging a balanced approach to treatment.

• A Master’s or Doctorate in Social Work, Mental Health Counseling, Marriage and Family Therapy, or Psychology from an accredited institution.
• Current licensure as an LCSW, LPC, or LMFT in the state of Tennessee.
• Demonstrated ability to address the mental health needs of individuals facing co-occurring medical conditions.
• Strong communication and interpersonal skills necessary to foster positive relationships with patients and interdisciplinary teams.
• Adherence to ethical guidelines, professional boundaries, and patient confidentiality protocols.
• Proficient in utilizing electronic health record (EHR) systems and other relevant software applications to track patient progress.
• A collaborative mindset and readiness to thrive as part of an interdisciplinary team dedicated to achieving the best outcomes for our patients.

• Prior experience in integrated care settings, particularly in outpatient facilities.
• Evidence of effective collaboration with medical professionals and multidisciplinary teams.
• Experience with specific patient populations, including those dealing with substance use disorders.
