About the Role
We are looking for a Care Coordinator who will play a key role in supporting patients after Physical Therapist or Advanced Practice Provider triage and throughout their episode of care. This individual will serve as the primary point of contact for patients, ensuring they understand and complete their recommended next steps, remain engaged, and move smoothly across services. The Care Coordinator will collaborate closely with Physical Therapists, Advanced Practice Providers, and other members of the care team to support referrals, follow-up, outcomes tracking, escalation when needed, and ongoing refinement of care coordination workflows as we scale. This role reports to the VP of Care Delivery.
What You'll Do
Patient Support
  • Serve as the primary point of contact for patients following Physical Therapist or Advanced Practice Provider triage.
  • Guide patients through recommended next steps and support throughout the episode of care.
  • Conduct proactive follow-up outreach to support adherence, re-engagement, and continuity of care.
  • Deliver a supportive, patient-centered experience aligned with RightMove standards.

Care Navigation
  • Manage care navigation and transitions across levels of care in coordination with Physical Therapists, APPs, and external referral sources.
  • Execute and track referrals, including confirmation and receipt of external documentation.

Coaching & Adherence Support
  • Use basic coaching techniques, including goal setting, accountability check-ins, and barrier problem-solving, to support adherence.
  • Reinforce care team education in clear, patient-friendly language.

Collaboration & Continuity of Care
  • Work closely with Physical Therapists, APPs, and other care team members to ensure coordinated, patient-centered care.
  • Escalate clinical questions, changes in condition, or concerns to the appropriate provider when indicated.

Operational Excellence & Workflow Development
  • Partner with leadership to evaluate, validate, and improve care coordination workflows and policies.
  • Identify process gaps or inefficiencies and propose structured solutions.

Documentation & Compliance
  • Document patient communication, follow-up, outcomes, and care coordination activities accurately and consistently in the electronic health record.
  • Collect, document, and maintain accurate patient-reported outcomes and engagement data to support clinical decision-making and value-based reporting.
  • Adhere to established workflows, escalation pathways, and quality standards within a value-based care environment.

Must-Haves
  • High school diploma or GED (associate degree or higher preferred).
  • Experience in healthcare coordination, patient support, coaching, or medical office roles.
  • Comfort conducting structured follow-up conversations focused on motivation, behavior change, and adherence, not solely scheduling.
  • Strong organizational skills with reliable follow-through and ability to operate within structured, evolving workflows.
  • Excellent written and verbal communication skills.
  • Ability to work effectively in a remote environment and manage digital workflows.

Preferred
  • Experience in telehealth, digital health, musculoskeletal care, or physical therapy settings.
  • Spanish language proficiency.
