• Develop and implement individualized treatment plans based on thorough clinical assessments, ensuring these plans align with the unique needs and preferences of each client.
• Participate actively in a multidisciplinary team, contributing insights and strategies that enhance the overall treatment plan and improve patient outcomes.
• Continuously monitor clients’ progress, providing ongoing support and adjustments to treatment as necessary to promote positive developments in their care.
• Engage with clients through counseling sessions, utilizing strong verbal communication skills to facilitate discussions that encourage personal growth and improvement.
• Collaborate with families, healthcare providers, and community resources to ensure comprehensive support for clients, particularly in navigating end-of-life care.
• Leverage coaching techniques to empower clients in making significant life changes and addressing their psychosocial needs.
• Document all client interactions and treatment plans within electronic health record systems, adhering to industry standards and regulations.

The ideal candidate will be committed to serving patients in a hospice care setting. This role requires sensitivity and understanding as you work with individuals and families facing terminal illness. • A valid LICSW licensure is required.
• A Master’s degree in Social Work from an accredited program is essential.
• Strong verbal communication skills, with the ability to effectively counsel and advise patients and their families.
• Analytical thinking and problem-solving abilities to develop effective, tailored treatment plans.
• Coaching skills and a patient-centered approach to help individuals navigate their challenges.

• A minimum of two years of experience in clinical social work or a related field, with a focus on providing support in hospice or end-of-life care settings.
• Experience working collaboratively in a multidisciplinary team aimed at providing holistic patient care.
• Familiarity with electronic health records systems and telehealth platforms to maintain accurate documentation and facilitate remote support.
