• Utilize advanced therapeutic modalities, including behavior modification and assertive training, to effectively address patients' psychosocial needs.
• Conduct thorough assessments through effective interviewing techniques, chart reviews, and consultations with healthcare teams, to understand patients’ and families’ comprehensive medical and emotional histories.
• Develop, implement, and continually revise individualized treatment plans, ensuring they reflect ongoing psychosocial evaluations, the unique strengths, needs, and cultural factors of patients and their families.
• Facilitate educational and therapeutic group sessions, documenting patient progress and engagement while evaluating their responses to therapeutic interventions.
• Collaborate closely with multi-disciplinary teams to integrate services and optimize care continuity for patients, enhancing their overall treatment experience.
• Maintain accurate and timely documentation of all services provided as per organizational protocols and regulatory standards.

The ideal candidate will work primarily with patients in hospice care, where sensitivity to end-of-life issues and emotional support is essential. You will engage with patients who are experiencing chronic illnesses and their families, addressing complex emotional responses and assisting them in navigating available care options. A strong focus on respect for cultural and spiritual values is paramount.

• A Master's degree in Social Work from an accredited program is required.
• Current and active licensing as a Clinical Social Worker (LCSW) in California.
• Proven ability to communicate effectively, both in writing and verbally, to diverse audiences.
• Demonstrated capacity for maintaining professional boundaries and fostering therapeutic relationships with clients.
• Proficient in documenting client interactions and treatment plans efficiently and within required timeframes.

• Experience working within hospice, palliative care, or similar settings is strongly preferred.
• Knowledge of community resources and services available to patients and their families in California.
• Experience in delivering group therapy or therapeutic interventions effectively.

The annual salary for this position ranges from USD 120,000 to USD 130,000, commensurate with experience and patient caseload.
