Job description

Transitional Care Coordinator (Remote)Help Patients Stay Healthy at Home and Avoid Unnecessary Hospital Visits

Spectrum Medical Partners is seeking a compassionate, motivated, and energetic Transitional Care Coordinator to join our growing team. In this role, you\'ll serve as the first point of contact for patients recently discharged from the hospital, helping connect them with our Home Visit Program and the support they need to recover successfully at home.

This position is ideal for someone who enjoys building relationships over the phone, is comfortable making outbound calls, and is driven by helping patients access valuable healthcare services. While this is not a traditional sales role, a sales-minded approach is highly beneficial, as you\'ll be educating patients on the benefits of our program and encouraging participation in services designed to improve outcomes and reduce hospital readmissions.

This is a fully remote/work-from-home position.

What You\'ll Do
  • Make outbound calls to patients to introduce and educate them about our Home Visit Program

  • Build rapport and encourage participation in transitional care services

  • Schedule patient appointments based on provider availability and geographic location

  • Enter and manage patient information within proprietary databases and healthcare systems

  • Submit required documentation to Primary Care Physicians and healthcare partners

  • Conduct follow-up calls to identify additional patient needs and support opportunities

  • Communicate with referral sources and community partners

  • Collaborate with providers and internal teams to ensure a positive patient experience

  • High School Diploma or GED required

  • Comfortable spending a significant portion of the day on the phone

  • Strong computer skills and ability to navigate multiple software systems simultaneously

  • Excellent written and verbal communication skills

  • Ability to multitask and stay organized in a fast-paced environment

  • Bilingual (English/Spanish) preferred but not required

  • Ability to work independently in a distraction-free home office environment
