We are seeking a dedicated and experienced Licensed Mental Health Counselor (LCSW, LMFT, LPCC) to join our team at Nurtured Mind Wellness and Therapy, Inc. This remote, full-time position is designed for a passionate mental health professional who is committed to providing exceptional care to clients through telehealth services. Funded with a competitive annual salary ranging from $120,000 to $130,000, this role places a strong emphasis on patient-focused mental health services.

Key Responsibilities
  • Conduct comprehensive assessments to understand each client's unique mental health needs and create personalized treatment plans.
  • Facilitate individual therapy sessions, as well as group and family psychoeducation and process groups, utilizing evidence-based interventions to promote healing and growth.
  • Collaborate with a multidisciplinary team to ensure a cohesive approach to treatment and coordinate effective discharge planning.
  • Maintain detailed clinical documentation and complete assessments, group notes, and compliance reports promptly, adhering to all relevant guidelines and regulations.
  • Monitor and assess clients' safety conditions continuously, proactively identifying and addressing potential risks.
  • Lead treatment planning meetings, regularly reviewing and updating plans based on client progress and feedback.
  • Implement post-discharge follow-ups to ensure continued support and resources for clients transitioning back into the community.
Patient Population / Client Focus

Your work will primarily focus on a diverse client base requiring varying levels of mental health support, including those facing issues such as anxiety, depression, trauma, and relationship difficulties. Qualifications
  • A valid license as a Licensed Clinical Social Worker (LCSW), Licensed Marriage and Family Therapist (LMFT), or Licensed Professional Clinical Counselor (LPCC).
