Key Responsibilities
  • Perform thorough assessments of clients to develop tailored treatment plans that address their unique challenges and goals.
  • Actively communicate with clinical supervisors, and collaborate with direct care staff, family members, and other stakeholders to monitor and share individual progress effectively.
  • Maintain a well-structured schedule of client appointments, including home visits when necessary, to facilitate ongoing support and intervention.
  • Conduct home visits and observations as required, while meticulously documenting findings to provide comprehensive assessments and evaluations.
  • Ensure all therapeutic programs are implemented properly, promoting the welfare and treatment goals of each patient.
  • Participate in Interdisciplinary Team Meetings to enhance collaboration and information-sharing across the caring community.
  • Prepare and complete monthly and quarterly progress notes to track client development and outcomes.
  • Lead training initiatives to assure staff members are proficient in implementing program plans and documentation practices.
  • Offer continued follow-up and support to clients through interviews and additional visits, helping to adjust plans as needed.
  • Maintain organized and accessible records that facilitate communication among team members and authorized external parties.
  • Serve as a liaison between the clinic and the mental health community to aid in developing effective program goals.
  • Assist in formulating updated training materials to enhance our programs continually.
Patient Population / Client Focus

Our center serves a diverse clientele consisting of adults and youth facing a wide range of mental health challenges. You will have the opportunity to work with individuals navigating issues such as anxiety, depression, trauma, and substance use, providing them with the tools and support they need for recovery and resilience.

Qualifications
  • A Master's or Doctoral degree in Counseling, Psychology, or a related field is required.
  • An active and unrestricted licensure as a Professional Counselor (LPC) in Pennsylvania.
  • A minimum of two years of counseling experience, preferably in a clinical setting, is required.
  • A strong foundation in various therapeutic approaches, particularly in family therapy, couples therapy, substance abuse, or trauma treatment.
  • In-depth knowledge of legal and ethical standards within counseling practices.
