• Conduct detailed assessments of patients’ mental health, focusing on emotional, behavioral, and social functioning by employing evidence-based techniques.
• Design and implement individualized treatment plans that are culturally competent and tailored to address each patient’s unique circumstances and challenges.
• Provide a spectrum of mental health services, including psychotherapy for conditions such as anxiety, depression, and PTSD, using therapeutic modalities that suit each client.
• Monitor ongoing patient progress through regular follow-ups, adjusting treatment strategies based on patient feedback and clinical observations.
• Educate patients and their families on mental health conditions, proposed treatments, and available community resources to empower them in their recovery journey.
• Collaborate with interdisciplinary teams, including psychiatrists and primary care providers, to ensure a cohesive approach to patient care and treatment integration.
• Maintain meticulous and confidential patient records using electronic health record (EHR) systems, documenting all interactions, assessments, and treatment outcomes.
• Uphold ethical and professional standards, ensuring patient confidentiality and informed consent processes are consistently followed.

The patient population you will serve encompasses a wide range of individuals dealing with various mental health issues, including but not limited to those facing anxiety, depression, trauma-related disorders, and behavioral challenges. Your role as an LCSW involves not only providing psychotherapy but also understanding the cultural and social contexts of each patient, thus fostering an inclusive environment that promotes healing and recovery.

• Master's degree in Social Work from an accredited institution.
• Current state licensure as a Licensed Clinical Social Worker (LCSW).
• A minimum of 2 years of experience in providing psychotherapy and mental health services, with a proven ability to handle a diverse client base.
• Exceptional communication and interpersonal skills, enabling effective collaboration with patients and healthcare professionals alike.
• Strong clinical competencies, including proficiency in conducting thorough assessments and developing actionable, evidence-based treatment plans.
• Experience with electronic health records (EHRs) and healthcare software for managing patient information and coordinating care.

• Commitment to confidentiality and adherence to ethical and professional standards in all interactions.

• Experience in a community health setting, particularly one that emphasizes remote care delivery.
• Demonstrated ability to work effectively in a team-based environment, contributing to interdisciplinary discussions and decision-making.
• Familiarity with therapeutic models that address trauma and culturally sensitive treatment approaches.
