• Conduct comprehensive assessments of clients' psychosocial needs through individual interviews, developing a deep understanding of their emotional, mental, and social requirements.
• Collaborate with clients and their families to create tailored care plans that meet their specific goals, considering both short- and long-term needs.
• Provide counseling and emotional support, facilitating discussions on life transitions, grief, and challenges that seniors face.
• Actively advocate for clients’ rights and access to community resources by navigating social systems and ensuring they receive necessary support.
• Work closely with interdisciplinary teams, including nurses, caregivers, and other healthcare professionals, to coordinate holistic care plans.
• Regularly monitor the well-being of clients through virtual check-ins, ensuring their living conditions are safe and conducive to their health.
• Facilitate effective communication within family dynamics, helping to resolve conflicts and build supportive environments for the client.
• Educate clients and families about community resources and support groups that enhance their social engagement and overall quality of living.
• Maintain thorough and accurate documentation of all client interactions, assessments, care plans, and progress notes, ensuring compliance with industry standards.
• Stay informed about current trends and practices in social work, implementing ethical standards in every aspect of your role.

Your primary focus will be seniors and their families, addressing a variety of psychological issues and life challenges. You will engage with individuals who may be experiencing anxiety, depression, loneliness, or significant life transitions, providing the guidance and support essential for fostering resilience and improving their quality of life.

• Master's degree in Social Work from an accredited institution.
• Current and valid state licensure as a Licensed Clinical Social Worker (LCSW).
• Minimum of two years of experience in a social work role, preferably within a community health setting or focused on geriatric care.
• Demonstrable knowledge of the principles of social work, particularly as they pertain to aging and mental health.
• Strong assessment, counseling, and problem-solving skills with a compassionate and empathetic approach to client interactions.
• Excellent communication and interpersonal skills to facilitate effective teamwork and client relations.
• Proficiency in electronic health record (EHR) systems and other computer applications relevant to client care.

• Experience working in community health clinics or geriatric-focused organizations is highly regarded.
• Familiarity with insurance policies and resources available for senior care.
• Additional certifications in trauma-informed care or mental health support strategies will be beneficial.

The salary range for this position is between USD 90,000 and 100,000 annually, commensurate with experience and qualifications.
