• Conduct comprehensive clinical assessments to evaluate the needs of patients and develop individualized treatment plans utilizing solution-focused methodologies.
• Implement evidence-based interventions and therapeutic techniques, catering to the diverse needs and backgrounds of patients in a hospice setting.
• Maintain accurate clinical documentation and patient records in compliance with local and federal regulations, ensuring transparency and accountability in care delivery.
• Collaborate with healthcare professionals, including physicians, nurses, and other social workers, to ensure a holistic approach to each patient’s care.
• Engage in regular case consultations and participate in peer review meetings to enhance clinical practices and improve patient outcomes.
• Provide psychoeducation and support to patients and their families, equipping them with the tools they need to cope with grief and transition.
• Stay informed about current research and best practices in social work, applying this knowledge to improve therapeutic interventions and patient care.

In this role, you will primarily work with patients facing terminal illnesses and their families, providing compassionate support during one of the most challenging times in their lives. Your interventions will focus on addressing emotional, psychological, and social needs, aiming to enhance the quality of life and provide peace of mind for both patients and their loved ones.

• A Master's degree in Social Work from an accredited institution.
• A valid LCSW or LSCSW license in the state of Kansas.
• A minimum of 2 years of clinical social work experience, especially in a healthcare or hospice setting.
• Expertise in solution-focused therapy and family systems theory.
• Exceptional communication skills, both written and verbal, to facilitate effective interactions with patients and healthcare teams.
• A proven ability to thrive in a collaborative environment while managing a diverse caseload efficiently.
