Position Overview

Key Responsibilities

As a Licensed Professional Counselor, you will:

• Assist primary care providers in recognizing and managing mental health and psychosocial issues as a crucial member of the primary care team.
• Conduct thorough patient intakes that include diagnostic evaluations, leading to informed treatment recommendations for primary care providers.
• Provide brief, focused therapeutic interventions tailored to patients’ mental health needs.
• Advise on which patients may be better served within the primary care system versus those needing referral to specialized mental health services.
• Initiate follow-up communications to assess patient progress and adapt treatment plans accordingly.
• Develop relapse prevention strategies and assist patients in maintaining long-term functioning stability.
• Monitor patients at risk, collaborating with providers to create preventive care plans.
• Document patient progress and treatment plans meticulously while ensuring all professional and ethical standards are adhered to.
• Empower patients with self-management strategies and education to actively engage in their recovery process.
• Provide consultation and insights on behavioral health services to team members and clinic management to enhance program effectiveness.

Patient Population / Client Focus

Your primary focus will be on adults and children facing various mental health challenges, including anxiety, depression, trauma-related issues, and behavioral disorders. You will work closely with individuals from diverse backgrounds while adhering to cultural competence and sensitivity, ensuring that all patients receive tailored care that respects their unique experiences and identities.

Qualifications

To be considered for this position, candidates must possess:

• A Master’s Degree in Clinical Counseling or a related field.
• A current and valid Licensed Professional Counselor (LPC) license in the state of Pennsylvania.
• Extensive knowledge of mental health assessment techniques, treatment planning, and psychotherapeutic methods, including family and group therapy.
• Understanding of developmental psychopathology and the social implications of mental health conditions on individuals and communities.
• The ability to effectively maintain patient records and prepare related documentation as per clinical standards.
• Strong communication skills to establish collaborative working relationships with various stakeholders.
• A flexible approach to patient care that accommodates individual needs and preferences.

Preferred Experience

While not required, the following experiences are advantageous:

• Proficiency in both English and Spanish, enhancing communication with a broader patient demographic.
• Prior experience working within a Community Health Center or similar clinical setting.
• Familiarity with Federally Qualified Health Center (FQHC) guidelines and productivity targets.
