Position Overview

The Second Chance Health Center is actively seeking a dedicated and experienced Licensed Clinical Social Worker (LCSW) to join our team in a full-time remote capacity. Key Responsibilities
  • Provide telehealth sessions to clients experiencing mental health challenges, ensuring the implementation of evidence-based practices and therapeutic modalities.
  • Conduct thorough assessments to determine the medical necessity for services and develop individualized treatment plans that reflect clientsโ€™ needs and goals.
  • Document client interactions and progress comprehensively to maintain accurate records in compliance with healthcare regulations.
  • Utilize your knowledge of community resources to help clients navigate complex systems such as food assistance, healthcare, and housing support.
  • Respond effectively to crisis situations, including mental health emergencies and cases of child abuse, while ensuring the safety and well-being of clients.
  • Monitor and evaluate clients' progress, following up regularly to adjust treatment plans as needed.
  • Facilitate referrals to additional resources, programs, or specialists to enhance the support network available to clients.
  • Assist clients in adjusting to various life challenges, such as job loss, health issues, and interpersonal conflicts, through ongoing counseling and support.
Patient Population / Client Focus

Your work will primarily involve assisting individuals facing significant life challenges, including veterans, those struggling with substance use disorders, and individuals affected by trauma. You will play a crucial role in empowering clients to reclaim their lives and develop resilience through mental health interventions.

Qualifications
  • Masterโ€™s Degree in Social Work (MSW) from an accredited institution is required.
  • A valid Licensed Clinical Social Worker (LCSW) license is mandatory to practice.
  • A minimum of two years of experience providing mental health services is essential, preferably within a healthcare-related environment.
Preferred Experience
  • Experience working with veterans and understanding the unique challenges they face is highly desirable.
  • Demonstrated expertise in trauma-informed care and techniques for mental health treatment.
  • Solid familiarity with community resources beneficial for client support, including referral processes.
Compensation and Benefits

We offer a competitive salary in the range of $120,000 to $130,000 annually, commensurate with experience and qualifications.
