THIS ROLE IS HYBRID ROLE. MUST RESIDE IN THE CHICAGOLAND AREA PREFERABLE COOK COUNTY FOR CONSIDERATION

  • Monday through Friday
  • 8:00 AM - 5:00 PM CST
  • Some Travel
  • Pay Range $37-$44 per hour


The Case Manager is responsible for conducting medical management and health education programs for customers on government health care programs. Accountabilities include gathering, analyzing and providing date for regulatory reports.


Requirements

  • An active and unrestricted RN license in the state of IL with 3 years direct clinical care to the consumer in a clinical setting
  • OR Licensed Professional Counselor (LPC), OR Licensed Master Social Worker (LMSW), which includes 2 years of clinical practice
  • 2 years of HIV experience
  • 3 years wellness or managed care experience presenting clinical issues with members/physicians
  • 3 years of case management experience within a hospital or MCO setting
  • Analytical experience including medical data analysis
  • Proficiency in Microsoft Suite Word, Excel, Outlook, and Teams
  • Bilinguel in Spanish and English preferred
  • Reliable transportation and valid car insurance
