• Conduct thorough virtual psychosocial assessments to identify clients' unique strengths and needs, which will inform personalized treatment plans.
• Provide evidence-based therapeutic services, including individual, family, and group therapy, utilizing advanced techniques tailored to various mental health issues.
• Collaborate proactively with interdisciplinary teams, encompassing physicians, nurses, and other healthcare professionals, to coordinate comprehensive care strategies that enhance overall client wellness.
• Advocate for clients’ rights and assist them in accessing various community resources and support services, ensuring they receive the assistance they need beyond therapy.
• Document all client interactions, treatment progress, and outcomes meticulously within our electronic health records system to maintain accurate tracking and reporting.
• Engage in regular team meetings, case reviews, and continuing education opportunities to expand your skill set and contribute knowledgeably to team discussions.

In your role as an LCSW at our hospice center, you will provide care for a diverse client base facing terminal illnesses, grief, and other psychological distress related to life-limiting conditions. Your focus will be on promoting emotional and psychological well-being, helping clients and their families navigate complex emotional landscapes, and facilitating meaningful conversations about end-of-life care.

• A Master's degree in Social Work from an accredited institution.
• Current licensing as a Licensed Clinical Social Worker (LCSW) in Arizona.
• A minimum of three (3) years of post-licensure experience in delivering clinical social work services, preferably in a hospice or medical setting.
• Demonstrated proficiency in conducting diagnostic assessments and crafting effective treatment plans for clients with significant psychological or emotional needs.
• Exceptional interpersonal and communication skills, with an ability to form strong relationships with clients from diverse backgrounds.
• A commitment to cultural competence, social justice principles, and ethical clinical practices.

• Experience in hospice or palliative care settings, with a focus on understanding the unique challenges faced by clients in this demographic.
• Familiarity with integrative care models and the ability to approach treatment from a multi-disciplinary perspective.
• Proven capabilities in presenting clinical findings and treatment plans to interdisciplinary teams.
