Elevate Therapy is seeking a compassionate and skilled Remote Licensed Clinical Social Worker (LCSW) to join our dedicated hospice care team. This full-time position offers a unique opportunity to provide essential mental health support to patients and their families in a hospice setting. This is a remote position based out of Salem, Oregon, enabling you to serve clients from the comfort of your own home while still being part of a closely-knit team dedicated to hospice care. We offer flexible scheduling to ensure that you can meet the needs of our clients while maintaining a healthy work-life balance. Some evening and weekend hours may be required to best accommodate patients' schedules.

• Conduct comprehensive psychosocial assessments to evaluate the mental health and emotional needs of hospice patients and their families.
• Provide individualized therapy sessions, including one-on-one, family, and group therapy, using a range of evidence-based therapeutic modalities, such as cognitive-behavioral therapy and trauma-focused therapy.
• Develop and implement personalized treatment plans that will address the unique goals and challenges of each client, adapting to their evolving needs as required.
• Collaborate extensively with a multidisciplinary treatment team, including physicians, nurses, and other mental health professionals, to ensure holistic and integrated care.
• Facilitate group therapy sessions and psychoeducation classes that empower patients and family members in understanding and coping with the emotional aspects of hospice care.
• Understand the specific protocol for risk assessment and safety planning to proactively address any crisis situations.
• Maintain accurate, timely documentation in adherence to ethical and legal standards, ensuring that all client interactions and treatment plans are well recorded.
• Participate in regular team meetings to discuss client progress and contribute insights towards improving care strategies.

You will be supporting a diverse group of patients experiencing a range of emotional and psychological difficulties associated with chronic illness and end-of-life transitions. This includes working with patients suffering from depression, anxiety, and trauma while also addressing the emotional impact on their families. Your interventions will be pivotal in providing comfort and clarity during a challenging period in their lives.

• A Master’s degree in Social Work from an accredited institution.
• Possession of an active Oregon state license as a Licensed Clinical Social Worker (LCSW).
• A minimum of two years of clinical experience in a mental health or outpatient setting, preferably with a focus on hospice or palliative care.
• Strong competency in various evidence-based therapeutic modalities tailored to the needs of individuals and families in a hospice environment.
• Excellent communication, empathy, and interpersonal skills that foster relationships of trust and respect with clients and colleagues.

• Experience working within hospice or palliative care settings is highly desirable, offering direct insight into the complexities of end-of-life dynamics.
• Familiarity with the documentation standards and treatment planning specific to hospice care.
• Knowledge of available community resources and support networks for patients and families navigating grief and bereavement.
