• Conduct thorough psychosocial assessments to identify clients' emotional support needs and key stressors related to their medical conditions.
• Utilize evidence-based therapy techniques, providing individual, family, and group counseling focused on grief, coping strategies, and emotional resilience.
• Work closely with interdisciplinary teams—including physicians, nurses, and chaplains—to deliver integrated care that addresses both emotional and physical health.
• Proactively advocate for clients to ensure access to necessary community resources, support services, and patient rights.
• Maintain meticulous documentation of client interactions and therapy outcomes using electronic health record systems, ensuring compliance with industry standards and regulations.
• Engage in regular team meetings and case conferences to discuss patient care strategies and receive feedback on treatment plans.
• Participate in ongoing training and professional development initiatives to maintain licensing requirements and enhance clinical knowledge.

This role specifically serves individuals and families within a hospice setting, addressing complex emotional and psychological needs that often accompany terminal illnesses. You will provide support to those navigating end-of-life challenges, engaging with clients who are experiencing grief, existential concerns, and the intricacies involved in palliative care.

• A Master’s degree in Social Work from an accredited program is required.
• Active licensure as a Licensed Clinical Social Worker (LCSW) in the state of California.
• A minimum of two (2) years of post-licensure clinical experience, particularly in a healthcare or hospice setting.
• Demonstrated proficiency in conducting diagnostic assessments and formulating effective treatment plans.
• Outstanding interpersonal and communication abilities, coupled with a strong sense of empathy and understanding for diverse populations.
• A commitment to cultural competence and ethical practice in all interactions.

• Previous experience in hospice care, palliative medicine, or similar environments is highly desirable.
• Exposure to interdisciplinary team collaboration, particularly in chronic illness contexts.
• Experience documenting client interactions in electronic health record systems, ensuring adherence to best practices.

• The compensation for this position ranges from USD 115,000 to 130,000 annually, commensurate with experience and caseload.
