• Conduct comprehensive psychosocial assessments to evaluate each patient's mental health status, strengths, and unique needs within the context of their hospice experience.
• Provide evidence-based therapeutic interventions, such as cognitive-behavioral therapy (CBT) and dialectical behavior therapy (DBT), tailored to the emotional states of terminally ill individuals.
• Collaborate closely with a variety of healthcare professionals, including psychiatrists, nurses, and spiritual care providers, to create a cohesive treatment plan that addresses both physical and mental health concerns.
• Assist patients and their families in understanding diagnoses, treatment options, and medication regimens, ensuring that they feel empowered to make informed decisions during challenging times.
• Facilitate individual, family, and group therapy sessions, applying evidence-based models that resonate with patients coping with loss and grief.
• Develop personalized treatment plans in collaboration with the interdisciplinary team, integrating patients' goals, preferences, and cultural considerations.
• Guide patients through the complexities of the mental health system, advocating on their behalf to ensure they receive appropriate care and resources.
• Lead therapeutic groups and psychoeducation sessions that promote coping strategies for patients and their loved ones, addressing common challenges faced during end-of-life care.

• A Master’s degree in Social Work from an accredited institution is required to ensure a high standard of practice.
• Possession of active state licensure as a Licensed Independent Clinical Social Worker (LICSW) is essential for performing in this clinical role.

This position offers a competitive salary range of $80,000 to $100,000 annually, along with a sign-on bonus.
