In this role, you will:

• Conduct comprehensive psychosocial assessments of patients and their families to identify mental health needs and tailor individual treatment plans.
• Implement therapeutic interventions, including solution-focused therapies, as well as group and individual counseling sessions.
• Maintain thorough, accurate, and confidential documentation in compliance with HIPAA regulations, ensuring all patient information is secure.
• Participate in interdisciplinary team meetings to discuss patient progress and adapt care strategies to enhance outcomes.
• Engage in ongoing education and training, integrating the latest evidence-based practices in mental health therapy into your work.

Your primary focus will be on patients receiving hospice care, many of whom are experiencing life-limiting conditions such as cancer, chronic illnesses, and advanced age disorders. You will offer critical support for managing anxiety, depression, grief, and other emotional challenges associated with end-of-life experiences. Utilizing a person-centered approach, you will create therapy plans that respect the unique histories and preferences of each individual, fostering improved emotional well-being during their final stages of life.

• A Master’s degree in Social Work from an accredited institution.
• An active Licensed Mental Health Therapist (LMSW) license in Nevada.
• Exceptional communication skills, allowing you to engage effectively with patients, families, and team members.
• A strong foundation in mental health principles and a passion for serving the hospice population.
• Ability to thrive in a remote team setting while managing time effectively and maintaining high standards of care.

While not required, candidates with a minimum of 2 years of experience in hospice care, palliative care, or similar environments will be favored. Experience with trauma-informed care and a familiarity with various therapeutic modalities will enhance your ability to connect with patients and provide tailored interventions.
