This position is fully remote, allowing you to work comfortably from your designated workspace while engaging with clients and colleagues through virtual platforms. As part of our interdisciplinary team, you will collaborate with physicians, nurses, and other healthcare professionals to provide cohesive support to our patients and their families in the hospice setting. • Collaborate with a multidisciplinary team, including physicians and nurses, to deliver holistic care plans tailored to each patient’s needs.
• Develop and implement customized treatment plans that track client progress toward mental health goals, utilizing evidence-based practices and assessments.
• Educate patients and their families about available mental health resources and coping strategies during end-of-life care.
• Conduct regular assessments and document client interactions accurately in electronic health records, ensuring compliance with reporting standards.
• Stay updated on current trends and best practices in mental health care, particularly those relevant to hospice settings, to enhance service delivery.
• Provide individual and family counseling, focusing on grief management and emotional support within the context of hospice care.

This role primarily focuses on supporting elderly patients and their families. Many clients may be coping with terminal illness and require compassionate assistance navigating the complexities of the end-of-life process. Your experience will help you address the unique challenges the patient population faces, including depression, anxiety, and emotional distress, fostering a supportive environment.

• A Master’s degree in Social Work from an accredited institution is a fundamental requirement for this position.
• A valid Licensed Clinical Social Worker (LCSW) license is mandatory to provide clinical social work services.
• Strong clinical judgment along with effective communication skills are essential for interacting with clients, families, and interdisciplinary team members.
• Demonstrated ability to develop treatment plans and facilitate family counseling, particularly in sensitive hospice settings.

• A minimum of 2 years of experience as a Licensed Clinical Social Worker.
• Previous experience working with senior populations or in hospice or palliative care is highly desirable.
• Familiarity with telehealth tools and electronic health record systems to enhance patient engagement and documentation efficiency.

The compensation for this role ranges from USD 100,000 to 110,000 annually, reflecting your experience and contributions.
