We're seeking a compassionate, motivated Registered Nurse (RN) who is passionate about patient-centered care and eager to be part of a collaborative, supportive care team. You'll work a dream schedule of Monday through Friday, 8am-5pm, and no major holidays. After training, you will enjoy a schedule of being in the office for four days/week, and work from home one day/week, should you choose.
Eligible candidates may also qualify for the HRSA Nurse Corps Loan Repayment Program, offering meaningful student loan repayment in exchange for service in an underserved community.
JOB SUMMARY:
We are a busy primary care and behavioral health clinic looking for a motivated Registered Nurse (RN) to assist our medical staff. The Registered Nurse (RN) will provide support to the health, wellness, and disease management of HealthFirst Patients.
DUTIES and RESPONSIBILITIES:
The Registered Nurse (RN) is responsible for nursing care at patient appointments
  • Obtaining vital signs and patient history
  • Providing individualized education to patients related to acute and chronic diagnosis
  • Triaging patient calls and schedules visits as appropriate
  • Administering oral and injectable medication and treatments within the scope of practice and with orders from providers. May distribute sample medication per provider orders.
  • May perform venipuncture blood draws and urine collection for lab work as ordered by the provider
  • Maintaining patient data and health information in the electronic medical record according to documentation guidelines
  • Generating patient-related reports, as needed
  • Participating in performance improvement programs and disease management programs.
  • Assisting in the coordination of well-child and immunization programs
  • Assisting in ordering and stocking clinical supplies
  • Completing transitional care management follow-up for patients with hospital admissions
  • Monitoring assigned AthenaOne Buckets for patient follow-up according to outline protocol, including patient portal messages.
  • Monthly participation on the Life Risk Safety Committee
  • Routine safety and risk assessment to support Just Culture.
  • Supporting care coordination efforts as requested by the primary care provider

Requirements
EDUCATION/EXPERIENCE : Graduate of a Nursing Education Program, Current unrestricted Registered Nursing (RN) License for the state of NH; BLS certification required. One year of nursing experience preferred.
