• Provide individual and group therapy to assigned clients, utilizing evidence-based practices to foster mental and emotional well-being.
• Conduct thorough assessments to develop comprehensive treatment plans that align with clients' unique needs and goals.
• Actively participate in interdisciplinary team meetings to discuss patient progress and coordinate care strategies.
• Facilitate crisis interventions when required, employing risk assessment techniques to ensure client safety and provide appropriate resources.
• Document client interactions and treatment outcomes diligently using our electronic health records (EHR) system in accordance with HIPAA regulations.
• Engage with clients and their families to provide support, education, and resources to navigate emotional challenges related to end-of-life care.
• Regularly revise treatment plans based on ongoing assessments and client feedback, ensuring that holistic care efforts lead to improved mental health outcomes.
• Maintain compliance with the ethical standards set forth by the NASW code of conduct and Wellmore Counseling Center policies.

The population we serve at our hospice center comprises individuals experiencing advanced illness and significant emotional distress, including anxiety, depression, and adjustments to terminal diagnoses. As a Licensed Clinical Social Worker, you play a critical role in navigating these challenges with clients, offering therapeutic interventions that honor their individual experiences, beliefs, and cultural backgrounds while providing essential support during life transitions.

• Education: A Master’s degree in Social Work (MSW) from an accredited institution is required.
• Licensure: An active LCSW license in the state of California.
• Skills: Exceptional verbal and written communication skills, with the ability to build rapport and trust with clients and colleagues alike.
• Strong organizational skills and proficient with various digital communication tools.
• A rich understanding of assessment, diagnosis, treatment modalities, and crisis intervention strategies tailored to diverse populations.

• A minimum of two years of clinical experience working with clients who face serious mental health issues, preferably in a hospice or Palliative Care setting, is highly desired.
• Familiarity with electronic health record (EHR) systems.
• Experience working collaboratively in multidisciplinary teams.

• Competitive annual salary ranging from $70,000 to $85,000, commensurate with experience and qualifications.
