• Conduct ongoing assessments of clients to identify their individual needs and goals to tailor a personalized treatment plan.
• Facilitate effective communication among supervisors, direct care staff, families, and other relevant stakeholders regarding client progress.
• Develop and maintain a schedule for client appointments, home visits, and observation sessions to monitor therapeutic progress.
• Coordinate home visits that adhere to the program’s requirements, including conducting chart reviews and staff interviews, while analyzing relevant data.
• Ensure the effective implementation of program plans, adjusting strategies as necessary to better serve clients.
• Participate actively in Interdisciplinary Team Meetings to collaborate on care strategies and share insights relevant to client treatment.
• Complete detailed monthly and quarterly progress notes that reflect client advancements, challenges, and modifications to care plans.
• Organize and conduct training sessions to ensure that staff are equipped to implement program protocols successfully.
• Provide ongoing support through follow-up interviews, home visits, and observations to assess the evolving needs of clients.
• Maintain comprehensive and accurate records to ensure that valuable information is accessible to relevant parties.
• Serve as a liaison between the public and mental health community to support the objectives and development goals of the organization.
• Assist in creating and refining training materials aimed at improving program delivery and outcomes for clients.

In this role, you will engage with a varied population, including both adults and youth, who seek assistance with a range of mental health challenges. Clients may present with issues such as anxiety, depression, trauma, and relational difficulties. Your expertise will enable you to address the unique needs of each individual, promoting healthier lifestyles through targeted therapeutic interventions.

• A Master’s or Doctoral degree in Counseling, Psychology, or a related discipline.
• Active and unrestricted licensure as a Professional Counselor (LPC) in the state of Texas.
• Minimum of two years of counseling experience, preferably in a clinical setting, working with diverse populations.
• Strong understanding of legal and ethical standards related to mental health practice.
• Exceptional interpersonal and communication skills, with the ability to build rapport with clients and families.
• Demonstrated ability to collaborate effectively within a healthcare team.
• Proficiency with Electronic Medical Records (EMR) software and related technologies used in mental health settings.

• Specialization or additional training in family therapy, couples therapy, substance misuse, or trauma-informed care is highly desirable.
• Experience working in a community mental health context, particularly in remote or telehealth settings, is advantageous.

We offer a competitive salary range of USD 90,000 to 100,000 annually, commensurate with experience and caseload availability.
