• Conduct thorough ongoing assessments to effectively address the therapeutic needs of patients, ensuring that care plans are tailored to individual circumstances.
• Facilitate open communication with supervisors, family members, and healthcare staff regarding patient progress, fostering a collaborative and supportive environment.
• Develop tentative schedules to optimize appointments, home visits, and progress monitoring, maintaining flexibility to meet patient needs.
• Carry out essential home visits coordinated for in-home observations, chart reviews, and staff interviews, providing holistic care.
• Oversee the implementation and effectiveness of treatment plans, making adjustments as necessary to ensure positive patient outcomes.
• Actively participate in interdisciplinary team meetings to discuss patient cases and share insights that contribute to comprehensive care.
• Complete detailed monthly and quarterly progress notes documenting patient advancements and required interventions.
• Organize training sessions to ensure that all staff members are competent in program execution and proper documentation practices.
• Provide ongoing support through follow-up interviews and home visits to ensure continued adherence to treatment goals.
• Maintain meticulous records that support data accessibility for teamwork and accountability.
• Act as a liaison with the community and mental health stakeholders, fostering relationships that benefit patient development.
• Contribute to the creation of innovative procedures and training materials to enhance program quality and efficiency.

In this role, you will work with a diverse range of patients, including adults and youth from varied backgrounds facing different mental health challenges. Your expertise will be critical in addressing issues related to trauma, substance misuse, family dynamics, and other complex psychological needs. By customizing treatment plans, you will make a meaningful impact on the lives of your clients and their support systems.

• A Master’s or Doctoral degree in Counseling, Psychology, or a closely related field is required.
• Active and unrestricted licensure as a Licensed Professional Counselor (LPC) in the state of Oregon.
• A minimum of two years of counseling experience, preferably in a clinical setting, is essential.
• Specialization in areas such as family and couples therapy, substance use treatment, or trauma-informed care is preferred.
• Exceptional understanding of legal and ethical standards in mental health practice.
• Strong interpersonal skills with an ability to build rapport with diverse clientele.
• Demonstrated ability to collaborate and communicate effectively within a healthcare team.
• Proficiency with EMR software and other related technologies that facilitate comprehensive patient care.

While a minimum of two years of counseling experience is required, candidates with additional training or certifications related to specific therapeutic modalities or special populations will be favored. Experience in teletherapy or remote counseling is also a plus, given our virtual setting and the nature of our service delivery.
