• Conduct thorough psychosocial assessments to understand and evaluate clients' mental health concerns, strengths, resources, and needs.
• Provide individualized therapy sessions, employing evidence-based treatment modalities such as cognitive-behavioral therapy, solution-focused therapy, and trauma-informed care.
• Develop, implement, and regularly update personalized treatment plans collaboratively with clients, outlining specific goals, interventions, and progress measures.
• Facilitate engaging group therapy sessions and psychoeducation classes to support skill-building and collective healing.
• Work closely with a multidisciplinary team of psychiatrists, psychologists, and other healthcare professionals to ensure an integrated approach to client care.
• Offer crisis intervention, risk assessment, and safety planning based on clients' immediate mental health needs.
• Maintain accurate and timely documentation in compliance with ethical and legal standards, using electronic record-keeping systems.

In this role, you will primarily support clients who are navigating emotional distress related to end-of-life issues, loss, and grief. Your efforts will promote healing and resilience among a diverse clientele, ranging from individuals to families working through the unique challenges presented by hospice care. You will play a pivotal role in their therapeutic journey, providing comfort, guidance, and support.

• A Master’s degree in Social Work from an accredited program.
• Active and valid state licensure as a Licensed Clinical Social Worker (LCSW).
• At least two years of professional experience in a mental health or outpatient setting, preferably working with hospice or palliative populations.
• Demonstrable knowledge of evidence-based therapeutic modalities and effective treatment approaches.
• Strong interpersonal, communication, and assessment skills to foster meaningful client interactions.

• Experience providing therapy in a hospice, home health, or similar support setting.
• Background in crisis intervention, grief counseling, or trauma-informed practices.
• Advanced skills in therapeutic modalities such as Dialectical Behavior Therapy (DBT) or Acceptance and Commitment Therapy (ACT).
