As a Licensed Mental Health Counselor, your key responsibilities will include:

• Providing psychological support through education and counseling to both patients and their families.

• Advocating for patients' rights and accessing necessary services that safeguard their well-being.
• Maintaining up-to-date knowledge on private and government payer networks to navigate the healthcare landscape effectively.
• Utilizing electronic medical records to monitor patient progress and document interventions consistently.
• Implementing care coordination strategies that may involve identifying patients at risk of readmission and taking preventive actions.
• Leading transition conferences with patients and families to discuss care plans and address any concerns.
• Reporting any suspected abuse or vulnerability in compliance with legal mandates.
• Engaging in advocacy efforts that underscore the ethical aspect of care while supporting patients' families during challenging times.

You will work directly with a diverse array of clients in need of mental health support, including those facing acute stress, chronic psychological issues, and terminal illnesses. The ideal candidate will possess:

• A Master's or Doctorate degree in clinical psychology, counseling, or social work from an accredited institution.
• An independent license to practice in the state of Oregon (such as LCSW, LMFT, LPC).
