• Conduct comprehensive assessments and develop personalized treatment plans tailored to the needs of clients facing serious illnesses, ensuring their psychological and emotional support is prioritized.
• Provide individual and family counseling sessions via telehealth, delivering timely interventions and coping strategies for managing grief and loss.
• Collaborate with nursing staff, doctors, and other healthcare professionals to devise holistic care approaches that benefit clients and their families throughout the hospice journey.
• Maintain accurate client records and documentation in accordance with best practices, ensuring compliance with the organization's ethical guidelines and relevant regulations.
• Engage in regular team meetings to discuss patient progress, strategize on treatment options, and enhance team synergy in support of our collective goals.

Your role will specifically focus on clients in a hospice setting, where you will deliver care to individuals experiencing severe emotional disorders and mental health challenges related to terminal illnesses. Understanding the sensitive nature of end-of-life issues, you will work to foster a supportive environment for both clients and their families, helping them navigate complex emotional landscapes with understanding and respect.

• A Master’s degree in Social Work from an accredited institution is required.
• Licensed Clinical Social Worker (LCSW) license.
• Expertise in mental health assessment, diagnosis, risk assessment, and crisis management related to the hospice population.
• Exceptional verbal and written communication skills to effectively engage with clients and collaborate with team members.
• Strong organizational skills with the ability to manage multiple cases concurrently while meeting deadlines.

A minimum of 2 years of hands-on experience in clinical practice, particularly in areas related to mental health, grief counseling, or hospice care, is preferred. Experience working in medically under-served communities and familiarity with interdisciplinary care models will be beneficial.

The position offers a competitive salary ranging from $70,000 to $85,000 annually, commensurate with experience and qualifications. Additional benefits include:
