Job description

We are seeking a Clinical Pharmacist to join our primary care organization. You will play a critical role in improving patient outcomes through effective medication management, patient education, and collaboration with our healthcare team. In this role, you will support the full physician panel, regardless of health plan coverage and may involve multiple clinic locations.

In this remote position, you will report to the Associate Director Integrated Clinic Pharmacist and will integrate the pharmacist as a member of the care team within the physician practice.

Your daily responsibilities will include:

  • Use evidence-based medicine and clinical judgment to evaluate the suitability of pharmacotherapy for high-risk patients with chronic conditions, including diabetes, hypertension, heart failure and COPD.

  • Execute clinical protocols as allowed per organization and state regulatory agencies.

  • Optimization of drug coverage benefits for appropriate care and reduced costs (preferred formulary products, lower cost alternatives and home delivery pharmacy).

  • Collaborate with physicians, nurses, and other healthcare professionals to optimize patient care.

  • Participate in high-risk rounds with a multi-disciplinary team to discuss patient progress and adjust treatment plans.

  • Excellent patient counseling skills and the ability to communicate complex medical information to patients and healthcare providers.

  • Disease state education for patients on appropriate use of medications and self-management.

  • Engage in quality improvement initiatives aimed at enhancing medication safety and efficacy.

  • Post-discharge medication reconciliation for high-risk transition of care patients.

  • Document all activity within clinic EMR and other required pharmacy or clinical management systems.

  • Support CMS Stars and HEDIS quality measures for successful clinic performance.

  • Maintain accurate documentation of clinical interventions and patient care activities.

  • Conduct in-service education and training sessions for healthcare staff and develop and share educational materials related to pharmacotherapy.

  • Stay current with advancements in pharmacotherapy and clinical guidelines.

  • Provide consultation to healthcare providers on complex medication management issues.

  • Respond to additional requests from the Primary Care Organization, providing support within the pharmacist's defined scope of practice to advance organizational goals.

  • Address and implement targeted initiatives or priorities as directed by the National Director or Associate Directors of Pharmacy, according to best practices and regulatory standards.

Minimum Requirements

  • Active and unrestricted pharmacist license in the state of Florida

  • Willingness to become licensed in additional states for market support

  • Pharm.D. or other advanced pharmacy degree

  • Minimum five (5) years of experience as a practicing pharmacist

  • Experience working in a team environment and supporting common goals

  • Proficiency using with MS Office programs such as Word, Excel and Power Point, Teams, Outlook, and troubleshoot and resolve basic technical needs within a remote environment.

Preferred Requirements

  • Clinical residency in ambulatory care or similar discipline

  • Board certification in geriatric pharmacy

  • Experience working in an ambulatory care/primary practice setting

  • Familiarity with electronic medical record (EMR) systems

  • Experience in managing drug therapy for patients with chronic conditions

  • Identify and implement innovative solutions for improved efficiencies and outcomes

  • Multilingual (preferably English/Spanish)

Workstyle: Remote

Location: Must reside within a 50-mile radius of one of the following locations:

  • Englwood Park

  • Orange City

  • West Oakland

  • West Orange

  • Plant City

Schedule: M-F 8-5 Eastern

Travel: 5% - Typically, one (1) time quarterly for market meetings, as needed by the market

Work at Home Requirements: To ensure Home or Hybrid Home/Office employees' ability to work effectively, the self-provided internet service of Home or Hybrid Home/Office employees must meet the following criteria: At minimum, a download speed of 25 Mbps and an upload speed of 10 Mbps is required; wireless, wired cable or DSL connection is suggested. In certain roles, the minimum recommended internet speed required by Humana may not be sufficient for business needs. Humana reserves the right to require associates to upgrade their internet service if necessary. Work from a dedicated space lacking ongoing interruptions to protect member PHI / HIPAA information.Travel: While this is a remote position, occasional travel to Humana's offices for training or meetings may be required.

Pay Range

The compensation range below reflects a good faith estimate of starting base pay for full time (40 hours per week) employment at the time of posting. The pay range may be higher or lower based on geographic location and individual pay will vary based on demonstrated job related skills, knowledge, experience, education, certifications, etc.


$126,300 - $173,700 per year


This job is eligible for a bonus incentive plan. This incentive opportunity is based upon company and/or individual performance.
