• Conduct comprehensive assessments of clients' social, emotional, and environmental challenges detailing their unique situations.
• Develop individualized service plans that include specific goals, interventions, and regular progress tracking to ensure effectiveness.
• Provide empathetic counseling support to individuals and families facing life-altering challenges associated with serious illness, grief, and loss.
• Engage collaboratively with clients by developing personalized service plans and connecting them with essential community resources, enhancing their quality of life.
• Facilitate crisis intervention and create risk management plans as required to ensure client safety and well-being.
• Team up with community agencies to synchronize services and advocate for client needs, ensuring they receive comprehensive care.

Your work will focus on clients facing severe health and social challenges, particularly those in hospice care. You will provide emotional and practical support to individuals and families dealing with terminal illnesses, navigating the complexities of end-of-life issues, and assisting them in finding peace and purpose during a challenging time. Your role is pivotal in helping clients access mental health resources, facilitate grief counseling, and offer emotional resilience strategies.

• A Master’s degree in Social Work from an accredited institution.
• Active licensure as a Licensed Clinical Social Worker (LCSW) in the state of Georgia.
• In-depth understanding of trauma-informed care practices and the psychosocial aspects of terminal illness.
• Exceptional communication skills and cultural competence to engage effectively with diverse populations.
• Capable of employing critical thinking and problem-solving skills in high-pressure situations.

• Previous experience working in a social service agency or similar environment, ideally in hospice care or mental health settings.
• Familiarity with the local community resources available to supporting vulnerable populations.
• Experience in case management and the development of comprehensive service plans.
