In this remote role, you will play an integral part of a community health clinic, guiding patients through the complexities of managing acute, chronic, or terminal illnesses. Work EnvironmentThis position is fully remote, allowing you the flexibility to work from your home while still being an essential part of our interdisciplinary team. You’ll collaborate closely with fellow clinicians and support staff via electronic communication platforms and integrated electronic medical records systems. Your ability to adapt to a virtual work environment and utilize technology efficiently will play a crucial role in maintaining effective communication and workflow.
Key Responsibilities- Conduct thorough assessments to identify patients' psychosocial needs and employ tailored interventions.
- Utilize an electronic medical record system to document patient interactions, track progress towards goals, and communicate effectively with multidisciplinary team members.
- Engage in regular communication with your team, anticipating needs and adjustments to care plans based on patient feedback and clinical developments.
- Educate patients and their families about available resources and provide support throughout transition plans, ensuring proper referrals are made when necessary.
- Maintain up-to-date knowledge on government and private payer resources to promote appropriate patient transitions amongst various levels of care.
- Advocate for patients’ rights, ensuring they have access to protective services and addressing any issues related to vulnerability or suspected abuse in accordance with legal mandates.
- Plan and facilitate transition conferences with patients and families, enabling smooth transfers of care.
- Leverage technology and clinical inquiries to identify patients at risk for readmission and implement strategies to mitigate this risk.
- Provide direct counseling and support to patients and families, fostering an open line of communication and understanding.
- Participate in other duties as required, contributing to the overall success and performance of the practice.
The patient population you will serve includes individuals navigating various stages of mental health challenges, including those managing acute stress, chronic illnesses, and terminal conditions. Your holistic approach will empower them to confront their circumstances with resilience and support.
Qualifications- A minimum of a Master’s degree from an accredited institution in a related field such as clinical psychology, counseling, or clinical social work (e.g., MA, MS, MSW, MFT, MHC, PhD, PsyD).
- Possession of an independent licensure in Oregon or the state where you reside (e.g., LCSW, LMFT, LPC).
- Proficiency in navigating electronic medical records and a strong comfort level with digital communication tools.
- Demonstrated ability to manage a diverse caseload and navigate complex healthcare environments with ease.
- A genuine passion for improving behavioral health care and making a difference in patients' lives.
- Previous experience working in a community health setting or with a diverse patient population.
- Exposure to interdisciplinary collaboration with healthcare professionals across various specialties.
- Background in care coordination and an understanding of transition processes within health systems.
