Work Environment

This remote work setting allows for flexibility and work-life balance while ensuring you are part of a collaborative outpatient team. You will connect with colleagues through virtual meetings, case conferences, and interdisciplinary teamwork, all focused on enhancing patient care. Although the role is primarily remote, you may occasionally visit our outpatient office as needed for team meetings or direct patient interactions.

Key Responsibilities
  • Conduct thorough mental health assessments that evaluate the interplay between psychiatric and medical conditions, tailoring interventions to meet patient needs.
  • Design and implement integrated treatment plans that align with both psychological theories and medical recommendations, ensuring a holistic approach to each patient's care.
  • Facilitate individual and group therapy sessions using a variety of evidence-based therapeutic techniques, such as cognitive behavioral therapy (CBT) and mindfulness practices, customized to each patient's unique circumstances.
  • Engage in regular collaboration with medical providers to ensure the seamless coordination of care, optimizing treatment outcomes for patients.
  • Offer immediate crisis intervention and support for patients experiencing acute mental health challenges, demonstrating a calm and resourceful demeanor in stressful situations.
  • Educate patients on the symbiotic relationship between mental health and physical well-being, empowering them to take an active role in their recovery journey.
  • Maintain accurate patient records and documentation in accordance with legal requirements and ethical guidelines, using our electronic health record (EHR) systems effectively.
  • Participate actively in team meetings and interdisciplinary care planning to foster a collaborative, learning-oriented atmosphere.
  • Stay abreast of the latest research and clinical best practices in integrated care to continually enhance your therapeutic skills and knowledge.
Patient Population / Client Focus

The patients you will serve often present with a complex interplay of mental health and chronic physical conditions, including but not limited to chronic pain, autoimmune disorders, and substance use disorders. Understanding their unique challenges will be crucial in developing effective therapeutic strategies that address both mental and physical health needs.

Qualifications
  • Master's or doctoral degree in Social Work, Marriage and Family Therapy, Mental Health Counseling, or Psychology from an accredited institution.
  • Current and valid licensure as an LCSW, LCPC, or LMFT in the state of Illinois.
  • Proven proficiency in addressing the mental health needs of individuals facing co-occurring medical conditions.
  • Prior experience in delivering integrated care and working collaboratively with medical professionals is highly desirable.
  • Excellent interpersonal and communication abilities that facilitate rapport with patients and team members alike.
  • Strong adherence to professional boundaries and ethical guidelines, ensuring the confidentiality and rights of all patients are preserved.
  • Competence in utilizing electronic health record (EHR) systems and other technology to streamline patient care.

Preferred Experience
