• Conduct virtual therapy sessions with individuals and groups, utilizing evidence-based practices tailored to meet the unique needs of hospice patients.
• Develop personalized treatment plans that address emotional, psychological, and social challenges faced by clients and their families during difficult transitions.
• Maintain open communication with multidisciplinary team members, including nurses and healthcare providers, to coordinate care and enhance outcomes for clients.
• Comply with regulations by maintaining accurate and thorough documentation of client interactions, treatment plans, and progress notes in our secure electronic health record (EHR) system.
• Engage in continuous professional development through training in hospice palliative care practices and attending relevant workshops and seminars.

In this role, you will primarily support clients and their families experiencing advanced stages of illness, requiring end-of-life care and guidance. You'll navigate sensitive discussions, providing compassionate support as clients reflect on their lives and make significant decisions related to their care. Your work will play a crucial role in alleviating anxiety, grief, and emotional distress, helping clients find peace during challenging times.

• Master’s degree in Social Work from an accredited program.
• Active Licensed Independent Clinical Social Worker (LICSW) license in the state of Massachusetts.
• A minimum of two years of post-licensure clinical experience, specifically in mental health or hospice-related settings.
• Proficiency in assessment and treatment planning, with a focus on addressing the multifaceted challenges of hospice patients.
• A strong dedication to providing ethical, inclusive, and culturally sensitive care.

• Experience in end-of-life counseling or hospice care.
