• Conduct comprehensive psychosocial assessments by performing interviews, observations, and medical record reviews to evaluate factors affecting patient recovery.
• Provide compassionate counseling to patients and families, addressing emotional well-being, adjustment disorders, and coping strategies as they relate to the recovery process.
• Collaborate closely with the rehabilitation team to design and implement individualized treatment plans, ensuring the inclusion of psychosocial needs in care delivery.
• Assist patients in accessing vital community resources, including support groups, vocational rehabilitation services, and housing assistance to enhance their quality of life.
• Educate families on available community resources and post-rehabilitation options that foster continued growth and support.
• Engage in proactive discharge planning to ensure smooth transitions for patients returning home or to other care settings, and follow up to assess ongoing needs.

Your primary focus will be supporting individuals who have faced significant health challenges, including those recovering from strokes, spinal injuries, orthopedic conditions, or neurological disorders. Each patient comes with unique emotional and social needs, and your role will be to navigate these complexities with care and professionalism. Through your interventions, you will play a crucial part in their rehabilitation journey, helping them pursue a successful reintegration into the community.

• A Master’s degree in Social Work from an accredited program is mandatory.
• Active and unrestricted state licensure as a Licensed Clinical Social Worker (LCSW).
• Strong clinical skills and a solid understanding of psychosocial factors impacting recovery, including adjustment issues and grief.
• Excellent communication skills, both verbal and written, to effectively engage with patients, families, and the interdisciplinary care team.

• Prior experience in a rehabilitation or medical setting is highly desirable.
• Understanding of therapeutic interventions aligned with person-centered care approaches.
• Familiarity with community resources and advocacy strategies to aid patients in their recovery journeys.

We offer a competitive salary ranging from $75,000 to $100,000 annually, commensurate with experience and qualifications.
