• Conduct thorough and compassionate assessments of clients’ mental health and psychosocial needs upon admission to hospice care, ensuring individualized treatment plans are developed.
• Implement evidence-based interventions, such as cognitive-behavioral therapy or solution-focused brief therapy, aimed at alleviating emotional distress in patients and their families.
• Maintain accurate and confidential clinical records in accordance with HIPAA regulations and organizational standards, ensuring compliance and ethical practice.
• Collaborate with a multidisciplinary team, including physicians, nurses, and chaplains, to provide holistic care and address the comprehensive needs of hospice patients.
• Educate patients and families about the emotional aspects of terminal illness, offering them the tools and support necessary to cope effectively during this challenging time.
• Stay abreast of the latest research and developments in mental health and social work practices to inform clinical decisions and interventions.

This role focuses on providing care to a diverse range of hospice patients who are experiencing end-of-life challenges. The LCSW will engage with individuals facing terminal illness while assisting their families in navigating the emotional turmoil associated with grief and loss. Understanding cultural sensitivity and individual patient backgrounds will be crucial in developing effective interventions that honor each client’s unique journey.

• A Master’s degree in Social Work (MSW) from an accredited institution is required.
• A valid LCSW license in California, signifying recognized competence and clinical proficiency.
• At least 2 years of clinical experience in a health care or hospice setting, demonstrating a deep understanding of social work practices.
• Proficiency in evidence-based psychotherapy and a commitment to providing compassionate and individualized care.
• Strong communication, interpersonal, and problem-solving skills to effectively collaborate with team members and support patients.
• Ability to think critically and adapt therapeutic techniques to best meet the needs of patients and families.

Experience working with terminally ill patients and their families, particularly in hospice or palliative care settings. Familiarity with electronic health records (EHR) systems is beneficial for tracking patient outcomes and maintaining clinical documentation.

• Attractive annual salary in the range of $100,000 to $120,000, reflective of experience and qualifications.
