Position Overview

The Enlightened Mind Counseling Center is seeking a skilled and compassionate Licensed Master Social Worker (LMSW) to join our dedicated team in Plano, Texas. This full-time position focuses on providing essential social work services to seniors, ensuring their emotional, mental, and social well-being. The ideal candidate will utilize their expertise to advocate for the needs of elderly clients and their families in a supportive, remote school setting.

Work Environment

This remote position allows you to connect with clients directly in a school setting, where you will facilitate programs geared toward seniors. You'll work closely with a variety of interdisciplinary teams, including caregivers and healthcare professionals, to ensure holistic support for our clients. Key Responsibilities
  • Conduct thorough psychosocial assessments to identify the emotional, mental, and social needs of senior clients.
  • Collaborate with clients and families to design personalized care plans that reflect their individual goals and preferences.
  • Provide counseling, emotional support, and guidance to seniors and their families navigating life transitions such as health changes or relocation.
  • Advocate for clients’ rights by facilitating access to community resources and ensuring they receive appropriate support services.
  • Work collaboratively with interdisciplinary teams to monitor client care and address any concerns related to well-being or living conditions.
  • Conduct regular virtual visits to assess the ongoing needs and safety of clients, adjusting care plans as required.
  • Facilitate communication among family members to promote collaborative care that respects the client's preferences and needs.
  • Educate clients and families on available community resources, support groups, and programs aimed at fostering social engagement.
  • Maintain accurate documentation of assessments, care plans, progress notes, and client interactions to ensure continuity of care and compliance with ethical standards.
  • Stay informed about the latest practices, policies, and regulations in social work to enhance service delivery.
Patient Population / Client Focus

This role focuses specifically on serving senior clients, often dealing with issues related to aging, health transitions, and the emotional challenges that accompany these changes. You will play a crucial role in improving their quality of life and supporting their families through difficult times.

Qualifications
  • Master's degree in Social Work from an accredited institution.
  • Current and valid state licensure as a Licensed Master Social Worker (LMSW).
  • A minimum of two years of relevant social work experience, preferably in community health or geriatric settings.
  • Demonstrated knowledge of social work principles, aging-related issues, and community resource availability.
  • Strong assessment, counseling, and problem-solving skills demonstrated through previous work.

The compensation package for this position ranges between $60,000 and $100,000 annually, commensurate with experience and qualifications.
