This remote position allows you to connect with clients directly in a school setting, where you will facilitate programs geared toward seniors. You'll work closely with a variety of interdisciplinary teams, including caregivers and healthcare professionals, to ensure holistic support for our clients. • Conduct thorough psychosocial assessments to identify the emotional, mental, and social needs of senior clients.
• Collaborate with clients and families to design personalized care plans that reflect their individual goals and preferences.
• Provide counseling, emotional support, and guidance to seniors and their families navigating life transitions such as health changes or relocation.
• Advocate for clients’ rights by facilitating access to community resources and ensuring they receive appropriate support services.
• Work collaboratively with interdisciplinary teams to monitor client care and address any concerns related to well-being or living conditions.
• Conduct regular virtual visits to assess the ongoing needs and safety of clients, adjusting care plans as required.
• Facilitate communication among family members to promote collaborative care that respects the client's preferences and needs.
• Educate clients and families on available community resources, support groups, and programs aimed at fostering social engagement.
• Maintain accurate documentation of assessments, care plans, progress notes, and client interactions to ensure continuity of care and compliance with ethical standards.
• Stay informed about the latest practices, policies, and regulations in social work to enhance service delivery.

This role focuses specifically on serving senior clients, often dealing with issues related to aging, health transitions, and the emotional challenges that accompany these changes. You will play a crucial role in improving their quality of life and supporting their families through difficult times.

• Master's degree in Social Work from an accredited institution.
• Current and valid state licensure as a Licensed Master Social Worker (LMSW).
• A minimum of two years of relevant social work experience, preferably in community health or geriatric settings.
• Demonstrated knowledge of social work principles, aging-related issues, and community resource availability.
• Strong assessment, counseling, and problem-solving skills demonstrated through previous work.

• Experience working in a remote environment or telehealth services, especially within a school or senior care context.

The compensation package for this position ranges between $60,000 and $100,000 annually, commensurate with experience and qualifications.
