Key Responsibilities

• Conduct thorough evaluations and assessments of children to identify their unique developmental needs.
• Participate actively in the development and implementation of Individualized Family Service Plans (IFSP), ensuring alignment with best practices and regulations.
• Facilitate regular tracking and monitoring of outcomes, adjusting strategies as needed to support developmental milestones.
• Provide educational support and counseling to families, equipping them with the skills and knowledge to foster their child's development effectively.
• Collaborate with a multidisciplinary team to implement evidence-based practices and respectful natural learning environments.
• Utilize a coaching model to empower families in reinforcing learning and growth within natural contexts.
• Adhere to all applicable federal and state guidelines, ensuring compliance with health and safety regulations.

Patient Population / Client Focus

This role is primarily focused on serving infants and toddlers with developmental delays and disabilities. You will be instrumental in providing early intervention services tailored to each child's needs. The support you provide will not only aim to enhance physical, cognitive, social/emotional, and communication skills in young children but will also lay the groundwork for their lifelong learning and development.

Qualifications

• A Master’s Degree or Doctorate in Social Work.
• Valid LCSW license issued by the Department of Public Health.
• Minimum of one year of experience in early intervention services for children.
• Strong organizational and time management skills to juggle multiple cases and maintain accurate documentation.
• Proficient communication skills, both written and verbal, to liaise effectively with families and team members.
• A valid driver's license is required for potential travel to sessions with families, depending on service needs.

Preferred Experience

• Experience in utilizing Natural Learning Environment Practices.
